Anal Cytology Predicts Anal Precancer In HIV-Positive Gay Men

UCLA Center for Clinical AIDS Research and Education is one of only a few US centers offering an anal cytology screening service

FINDINGS: HIV-positive men who have sex with men are up to 90 times more likely than the general population to develop anal cancer. Detection of precancerous changes (anal dysplasia) by anal cytology - essentially an anal canal Pap smear - is a relatively new procedure and one that has yet to enter standard practice.

This UCLA study demonstrated that abnormal anal cytology was highly predictive of anal cell abnormalities that were subsequently confirmed by anal biopsy. The study was based on data from 244 patients at the UCLA CARE clinic who had anal cytology screenings between February 2002 and December 2004.

IMPACT: Abnormal anal cytology in HIV-positive men who have sex with men is highly indicative of the presence of abnormal anal cells that may be precancerous and should prompt further investigation. The UCLA CARE Center is one of a handful of clinics in the United States offering an anal dysplasia screening and management service.

The Chin-Hong studies of 2004 - 2005 told us that 95% of HIV+ gay males were infected with anal HPV;

and that 57% of HIV neg gay males were infected with anal HPV.

Giving a grand total of 67% of gay males infected with anal HPV.

That's probably every gay male who's engaged in receptive anal.

Because HPV is highly infectious.

Unlike HIV, where you might luck out now and then, chances are that if you're exposed to anal HPV, you get anal HPV.

And as Chin-Hong explains, the risk-factor for anal HPV among gay males is being anally receptive:

In multivariate analysis the investigators found that the only factor with a statistically significant association with HPV infection was receptive sexual intercourse in the previous six months (p < 0. 0001). The number of male sexual partners in the previous six months was of border-line significance (p = 0.06).

How do the numbers of gay males infected with anal HPV compare to the numbers of women infected with cervical HPV?

Well, a recently released study in JAMA revealed that about 27% of American women are infected with HPV.

That compares to 67% of gay men.

However, there's another figure which is far more telling.

And that's the percentage infected with "dangerous" or high-risk HPV.

Because only a few strains are high-risk for cancer.

There the numbers are quite different.

3.4% of American women are infected with one or more of the high-risk strains which the new vaccine Gardisil will protect against.

But 31% of gay men are infected with dangerous strains.

10 times the number!!!

Which of course tells you that they're doing something very dangerous.

It's not just that gay men are more promiscuous.

Remember what Chin-Hong said:

the only factor with a statistically significant association with HPV infection was receptive sexual intercourse in the previous six months

It's the Anal.

That's the problem.

Which is yet another about HIV predator Michael Neal in Australia, whom a psychiatrist deems the "most evil man" he's ever met; but whose behavior, I insist, is by no means unique or even unusual among gay males:

SUMMARY: Alleged HIV predator Michael Neal is the "most evil man" he's met in 20 years, according to a psychiatrist hoping to lock him up for life.

A psychiatrist who treated alleged HIV predator Michael Neal described him as the "most evil man" he had encountered in 20 years, a court in Melbourne, Australia, was told during Neal's committal hearing.

In 2004, psychiatrist Fintan Harte urged health authorities to "lock this man up for life" as the only way to guarantee the public health was not at risk.

Harte expressed concern about Neal having unprotected sex with other people in July 2004 and believed therapy would have little effect, if any, Elizabeth Hatch of the Department of Human Services told the court Thursday.

"He stated that Michael Neal enjoys infecting people with HIV and that he cannot see Mr. Neal maintaining safe sex with others," Hatch, a registered nurse who worked on Neal's case, said in a statement to the court. "Dr. Harte also said the only way to guarantee the public's health would be to lock this man up for life."

Ms Hatch also told the court that Neal, now 48, asked the department to pay for Viagra in 2003 because he found it difficult to get an erection with a condom on.

The court heard news of repeated attempted warnings regarding Neal's alleged sexually predatory behavior, including eight complaints to health authorities in Victoria state about his sexual behavior over five years, in addition to damning revelations concerning Neal's intent to infect men with the HIV virus.

Neal claimed to have organized a "conversion party" in 2005 at which up to 15 men had sex with a 15-year-old to infect him with HIV, the court was told Wednesday.

A witness told the court that Neal claimed to have injected a 15-year-old boy with crystal meth before he was gang-banged by up to 15 men at his Coburg home in 2005. The witness told the court Neal told him the boy wanted to contract HIV and that his brother would want to go through the same process.

Another witness appearing before the court, who is HIV-positive, said Neal tried to infect him with the virus in 2000. The witness told the court there was a movement of "bug chasing" and "gift giving" in the gay community -- terms used to describe the deliberate transmission of HIV.

The man said Neal looked triumphant when he asked him how it felt to "take a big pos load" when they had unprotected sex in 2000.

"There was absolutely nothing arousing or erotic about what he said. It was more a statement of violence," the man told the court.

Frances has written me about this case, saying,

I wonder if filthpig [Mr Neal's internet handle] grew up Catholic. Who knows. He wasn't lacking in aggressiveness, so that part of his masculinity is intact. But, it is a perverse form of aggressiveness turned against every male he could lay his hands on. It reminds one of the quotation "I am become death." It would seem that he took in the violence turned on him and accepted the verdict, then projected it outwards with a feeling of "any like me, I lay waste to." So, gay culture seems plenty aggressive to me. Thus the fantasy of fucking a "straight/masculine" guy for the sexual thrill of having him surrender his manhood, or I guess being fucked by one for purposes of self-loathing that have become completely sexualized.

In response, I said to Frances that gay culture isn't aggressive -- it's violent.

There are gay males who complain to me and / or Naked Wrestler aka NW about the "violence" on this site.

Which isn't violence.

It's natural male aggression.

As NW said in a recent email,

Why do fighters hug after a submission fight? Because they're showing affection (and respect)/MAN to MAN attraction after MAN to MAN AGGRESSION. They're normal guys.

They don't act or treat each other like girls or pseudo-women. They act and treat each other like Men. Men are Warriors by nature. The nature of the Warrior is also to be attracted to his fellow Warrior.

That's exactly right.

Men who are part of the Warrior group don't try to destroy each other after all.

Whereas the nature of the analized gay male is to act destructively and violently torwards his fellow gay male.

So when Frances says of Mr Neal aka filthpig that he's not "lacking in aggressiveness, so that part of his masculinity is intact" --

I disagree.

He is lacking in aggressiveness, and his masculinity is NOT intact.

His Natural Masculinity has been destroyed.

The extent of his "aggressiveness" is to stick his dick up a willing "victim's" ass.

Which I suppose in some quarters will pass for "social masculinity."

That is, a pseudo-masculinity which is delimited by society.

Social masculinity is pseudo-masculinity.

It is NOT Natural Masculinity.

And what Mr Neal is doing is NOT true or natural male aggression.

It's violence -- and typically criminal violence, which puts him at no risk.

Criminals in general seek that sort of violence -- violent for the victim, NO RISK for themselves.

That is NOT Manly or Masculine behavior.

Truly Masculine, Manly aggression is bound up with Fighting.

A strenuous physical *contest* -- what the Greeks called an "agon" -- thus our word "agony" -- with another man.

A fair fight -- based on considerations of weight and age.

And "one man's strenuous physical effort to overcome another" is also the definition of Greek athletics.

The Greeks didn't do team sports.

Not at least at the big religious-athletic competitions like the Olympics.

It was all one-on-one.

filthpig isn't a fighter

He's a victimizer.

Cultures which seek to develop Natural Masculinity among their men encourage men to be aggressive -- to fight -- within a socially acceptable context which benefits the men and the group.

Not just Sparta, but every Greek city-state trained their young men to fight.

From boyhood on, they trained in wrestling at the palestra, which was found in EVERY Greek city-state, and then from the ages of roughly 18 to 20 they had formal military training.

Which was obligatory.

We used to do that too by the way.

We called it "the draft."

And I think some form of obligatory military service is an important component of a true citizen-state.

Sparta was different in that boys were trained almost exclusively in fighting, warcraft, and survival skills for 23 years -- from the ages of seven to 30.

But every Greek city-state made sure that their young men knew how to FIGHT.

Because they understood what NW has said:

Frances also said, "Thus the [gay male] fantasy of fucking a "straight/masculine" guy for the sexual thrill of having him surrender his manhood"

As I explained to Frances, the goal in fucking a straight-identified guy isn't just to get him to surrender his manhood, it's to "turn him," to make him permanently "gay."

That's not so much a fantasy as it is an oft-stated goal.

You'll hear gay men say of a straight-identified guy, "He could be turned very easily."

For example, Patrick has a relative who's gay-identified.

He's also HIV poz, he carries hep C, he's a thief, and a barebacker, and he threatened to "turn" one of Patrick's karate students.

Patrick told him if he touched the kid, Patrick would kill him.

Which coming from Patrick is a credible threat.

Nonetheless, this particular gay male, like Mr Neal aka filthpig, will infect a man if he can.

So that's a reality, and that's why I feel that Neal is not so exceptional.

And Neal talks about infecting men in violent terms:

There used to be a song you heard at gay pride events and AIDS marches which had the lines,

"We are gentle, gentle people
fighting for our lives."

Is that true?

No.

First of all, it's not remotely that simple.

The core sexual act of the gay male community is anal penetration, and it's not gentle.

It's violent, and leads to the sort of explicit violence we hear about with people like Mr Neal.

Then there's the violence of S&M, which can get very violent indeed:

3 Sentenced In Castration 'Dungeon'

(Waynesville, North Carolina) Three men accused of operating what police described as a sadomasochistic "dungeon" that included castrations have been sentenced to jail time.

Richard Peter "Master Rick" Sciara, his partner of 20 years Michael Mendez, and the man they called their slave, Danny Carroll Reeves, pleaded guilty to felony castration and maiming. Superior Court Judge Dennis Winner said it was difficult to call the dungeon's willing patients "victims," but he said six castrations performed there were certainly a crime.

"I think this is a type of perversion that cannot be tolerated by society," Winner said during a sentencing hearing.

Prosecutors said the men ran a sadomasochistic "dungeon" fashioned from an enclosed carport in 2004 and 2005 at a house in a quiet neighborhood near Waynesville in western North Carolina. Six men, some from as far away as South America, came to the home for castration, while others went seeking other types of body-modification surgery, prosecutors said.

One has to wonder just what was modified.

And how permanently.

What you can see here, beyond the "we are gentle, gentle people," is that there's a passivity which is encouraged among gay males.

And which has deadly results.

I first ran into that in the late 1970s and early 80s, when I was trying to promote self-defense and martial arts among gay men.

And they simply wouldn't do it.

Even though in my city, NYC, at that time, fag-bashings were a weekly and sometimes daily event.

And that attitude has not gone away.

Four Guilty In Kevin Aviance Gay Bashing

(New York City) Four men charged in the brutal homophobic assault on gay entertainer Kevin Aviance last summer pleaded guilty in a Manhattan court Wednesday.

Avaince was attacked as he left the Phoenix bar last June. The four beat him unmercifully, breaking his jaw, doing serious damage to one leg and leaving him with cuts and bruises over most of his body.

As they attacked him the four young men yelled homophobic slurs.

Days later, with his jaw still wired shut and his leg in a brace, Aviance made a surprise appearance at a rally against homophobic violence.

"You can't keep a good queen down," Aviance told the crowd. But he warned against meeting violence with violence.

"We can't fight any of these people with arms and bullets and drama. We have to fight all these people with love, every single day," he said.

"We can't fight" says Mr Aviance.

Not true.

He's been taught that and has incorporated it into his performer's persona.

But that's not true.

"Meeting violence with violence" is a reasonable and usually successful strategy in dealing with the sort of street attack Mr Aviance suffered.

Fact is, people who know how to fight are far less likely to be attacked in the first place, and if they are attacked, are far more likely to survive.

Naked Wrestler:

If "gay guys" could just go to a fight school/grappling club on a Friday night instead of some cigarette/alcohol bar they might turn into real men again.

Some of the best times I've had were 2 hour sessions of kick boxing and pankration practice at the fight school on a Friday night, with other dudes who didn't have dates that night.

NW is telling the truth in both instances.

If gay males would begin to reclaim their masculine aggression, they would begin to become MEN again.

And nothing in life is sweeter than sparring -- on a Friday night or any other night.

But the "gay male community" under analism will have nothing to do with such activities.

They've been taught that "aggression" is bad.

And the beginning and end of their notion of self-defense is to use a condom -- sometimes -- while getting brutally fucked up the ass.

Fact is, analism breeds monsters, and they're tolerated by their community.

For example, most of the prosecutions for intentional infection of another person with HIV have been hetero:

Twin Falls resident Matthew Milligan, 37, was charged in 5th District Court last week after two women reported having unprotected sex with him. Both women told police Milligan never warned them that he was HIV positive. ...

It seems as though women are more likely to bring a complaint than are gay males, who've been taught it's their lot in life to get fucked, and that if they get infected, it's their fault for failing to "protect" themselves.

Again, those are core beliefs of both analism --

"you're a homo, get used to it"

"get on your knees bitch and have some fun"

-- and of the "safer-sex establishment," which has long argued that it's the responsibility of the negative individual to protect himself.

Which is like saying that it's the responsibility of the woman with the baby carriage in the crosswalk to protect herself from the drunk driver careening out of control.

NO it's not.

Which is why Idaho has a law which requires that someone who's poz notify his partner.

A court in California has said the same thing and gone farther -- if you have reason to believe, by reason of "lifestyle," that you *may* be infected, you MUST tell your partner.

So why aren't more gay men filing complaints and suing the guys who infect them?

Doing that would do more to lower infection rates than any condom campaign anywhere.

Because the one thing these guys care about is their money.

Here's something which we know will NOT work:

Interview: Feelings matter in curbing HIV

WASHINGTON, March 23 (UPI) -- United Press International interviewed Celia Lescano, a researcher at Bradley Hasbro Children's Research Center and lead author of a recent study on HIV transmission in adolescents and young adults -- who now account for half of new HIV cases each year.

Without helping young people manage the feelings and stress around sexual encounters, simply telling them how to use a condom is not enough, the article says. If the stress goes unmanaged, adolescents may not be able to effectively negotiate for safe sex with their partners.

...

Q: From a broader perspective, what do your findings tell us about our overall approach to HIV prevention?

A: The acquisition of HIV is quite complicated. Someone may see it as "Well, that person just didn't use a condom and that's how they got HIV, or that person didn't use a clean needle and that's how they got HIV." But the way we should envision it is that it's much more complicated. There are feelings that go into it. There are attitudes: how self-efficacious you might feel about your behavior and aspects of your relationship.

There's an emotions piece. It's not as simple as, "You need to use a condom, here's one, go use it." It really is, "Here's a condom and let's talk about how you might feel bringing up using this condom with your partner."

That's not true.

This is the sort of silliness which passes for safer-sex education and "COUNSELING" which we've been hearing for more than TWO DECADES.

I don't know why UPI decided that this person's article was news.

It's not.

A common AIDS "prevention" acronym is CNN.

CNN = condoms + needles + NEGOTIATION;

and NEGOTIATION has been the gold standard in condom campaigns for 20 years.

I repeat, NEGOTIATION, including "how you might feel bringing up using this condom with your partner" has been taught for 20 years.

I remember the very first "safer-sex" community talk I went to -- out on Fire Island on a warm summer evening in the mid-80s.

The presenter, who was a friend of mine -- he died a couple years later -- had a couple of zucchini and some condoms.

But he didn't just demonstrate how to put the condom on the zucchini, he talked about the emotional context in which one would NEGOTIATE using the device.

That was MORE than 20 years ago.

And still people get infected.

This is not a question of feelings.

It's a question of risk behavior and responsibility.

One way to address that is through the law.

Another way to address it, though, is by getting rid of the anal, and of the promiscuity and effeminacy which feeds on and results from the anal:

And setting up the opposite feedback loop:
